Bequest Language
A bequest is generally a revocable gift, which means it can be changed or modified at any time. You can choose to designate a bequest for a general or specific purpose so your gift will be used as you intend. Below are suggested phrases that may serve as a guide in selecting the type of gift you wish to make to IU Health. The sample language below is intended as a model for you to share with your legal adviser when your will or revocable trust is prepared and/or amended.
1. Specific BequestIf you are considering making an outright bequest to IU Health Foundation, we recommend the following language:
I hereby give, devise and bequeath _________ and No/100 dollars ($DOLLARS) to IU Health Foundation, a nonprofit organization located at 1633 N Capitol Ave - Suite 1200, Indianapolis, IN, 46202, Federal Tax ID #____________, for IU Health Foundation's general use and purpose.
Bequest of Specific Personal Property
Thank you for thinking of IU Health Foundation as the recipient of your personal property. Unfortunately, there is personal property we are unable to accept. Please contact us prior to leaving personal property to IU Health Foundation in your will or trust.
Bequest of Specific Real Estate
I hereby give, devise and bequeath all of the right, title and interest in and to the real estate located at ADDRESS OR DESCRIPTION OF PROPERTY to IU Health Foundation, a nonprofit organization located at 1633 N Capitol Ave - Suite 1200, Indianapolis, IN, 46202, Federal Tax ID #_______________, for IU Health Foundation's general use and purpose.
If you are considering making a bequest of a percentage of your estate to IU Health Foundation, we recommend the following language:
Restricted Bequests
If you are considering a bequest but would like to ensure that your gift is used for a specific purpose, please contact us. We would be happy to work with you and your attorney to identify ways to give and meet your charitable goals.
If you are making a restricted gift, we recommend your attorney includes the following provision to give IU Health Foundation flexibility should it no longer be possible for IU Health Foundation to use your gift as you originally intended:
